Distinguished Service Citation

Nomination Form

Each year, the University of Kansas and the KU Alumni Association are proud to recognize alumni and University friends with outstanding achievements “for the betterment of society and in behalf of humanity.” Recipients are selected from nominations submitted to the Distinguished Service Citation Selection Committee. Nominations may come from any source and need to include the nominee’s curriculum vitae or bibliography.

The Association will accept nominations for the 2008 Distinguished Service Citation Award through Sept. 30, 2008. Nominees selected to receive the award, the highest honor given by the University and the Alumni Association, will be recognized in the spring at the All University Supper and have the option to participate in Commencement as honored guests.

This form along with an attached letter from the nominator describing how the nominee has made a significant contribution “for the betterment of society and in behalf of humanity.” An attached current curriculum vitae for this nominee. Three letters supporting the nomination. These letters may be sent separately from the nominator’s materials and addressed to the Distinguished Service Citation Selection Committee at the address below.
